Community Based Patient Support Specialist - Mobile Health Unit Talbot & Dorchester County - 10 Month

Job Summary: The Community Based Patient Support Specialist is part of a cross-trained team providing support to the Community Based Department, including School Based Medical and Dental, WIC Fluoride, and Migrant programs. Performing a range of clinical, administrative, and staff support to multiple School Based Medical, Dental, Migrant, and administrative sites. This job requires traveling to various sites to function as support for the Community Based Medical & Dental providers specifically in CCHS Mobile Health Unit (MHU). Ensures that the SBHC, SBDP, Migrant, and other Community Based programs operate efficiently and effectively. This includes but in not limited to patient check in/out; vital sign entry; dental charting; correspondence to internal/external entities; billing and account maintenance, the ordering of supplies and equipment, communication with community partners, data collection, training, and billing. The Patient Support Specialist will assist with the preparation of marketing resources for outreach activities both within CCHS and in the community.

Required Skills/Abilities:

  • A valid driver’s license, driving CCHS vehicle, specifically our MHU is a requirement.
  • Bilingual language skills (Spanish/English) are a plus.
  • Will regularly travel to all school and community locations where CCHS provides care.
  • Highly organized, self-motivated, able to prioritize and multi-task.
  • Ability to work with diverse groups/individuals.
  • Computer skills including electronic health records and Microsoft Office.
  • Skilled in the development of brochures, fliers, newsletters
  • Extensive software skills, as well as Internet research abilities
  • Excellent interpersonal and communication skills.
  • Excellent problem-solving skills needed.
  • Familiarity with basic medical & dental terminology
  • Must possess excellent interpersonal skills and communicate effectively.
  • Demonstrated experience providing exceptional customer service consistent with AIDET.

Education and Experience:

  • High School Diploma or equivalent
  • Medical, Dental, or Behavioral Health office experience preferred including posting charges and payments.
  • Knowledge of CPT & ICD-10 coding.
  • Certified Nursing Assistant, a plus.
  • Proficient in basic math skills
  • Knowledge of electronic medical record software is helpful.

Duties/Responsibilities:

  • Models AIDET with all patient interactions and reinforces AIDET principles with staff.
  • Adheres to and models CCHS Standards of Behavior.
  • Provides training to new support staff and oversees continued training for current staff, when necessary, on topics such as posting, changes to insurance plans and other related issues.
  • Prepare, distribute, collect, and maintain SBHC/SBDP enrollment forms each school year.
  • Prepare chart, document, and obtain supporting paperwork for medical/dental providers.
  • Stock, maintain and operate the Mobile Health Unit (MHU)
  • Supports day-to-day activities of the Community-Based programs.
  • Assists in developing and/or updating all program materials.
  • Assists community-based program staff to meet timelines regarding posting, preparation of paperwork and follow up.
  • Assists with the organization of service data.
  • Serves as a liaison with billing office.
  • Updates the Practice Management System to accurately reflect patient demographics and insurance information.
  • Participates in Back-to-School nights and other related activities to promote the SBHC.
  • Communicates with school personnel, CCHS Administration and co-workers for schedule changes, closings, etc.
  • Answers telephone promptly in a professional manner and documents phone messages in the Practice Management System.
  • Schedules patient appointments in the Practice Management System and coordinates the patient flow.
  • Ensures that patient visit report is sent to student’s primary care provider and to appropriate MCO.
  • Sends correspondence to student’s parent/guardian regarding SBHC visit.
  • Scans chart information into Practice Management System.
  • Checks and follows up on tasks sent in the Practice Management System.
  • Utilizes the EVS system to verify patient eligibility.
  • Balances work at the end of the day and closes journal.
  • Collects school staff copays and submits payments to administrative office.
  • Posts charges into Practice Management System within 72 hours of date of service
  • Promotes enrollment in the Maryland Children’s Health Insurance Program.
  • Maintains inventory, rotating stock for cost efficiency, and submits inventory list in writing to administration each month.
  • Accurately documents and updates logs per SBHC policy and procedures.
  • Cleans nebulizer masks and tubing and stores in a labeled bag for SBHC asthma patients.
  • Cleans and straightens exam room after each patient and follows CCHS Cleaning and Disinfecting policy.
  • Follows CCHS policies regarding storing/locking medicine and chart cabinets.
  • Removes expired medication/supplies from SBHC monthly.
  • Follows school security procedures.
  • Prepares chart (as needed), supplies and supporting paperwork for provider.
  • Obtains vitals as appropriate.
  • Creates LabCorp requisition slips and specimen labels.
  • Transports lab specimens to the nearest CCHS Office in a timely manner.
  • Regular, reliable attendance is a requirement of this job.
