Job Summary:

The primary role of the Patient Outreach Specialist is to educate Choptank patients regarding the appropriate use of a medical home to promote optimal health by bringing patients into the centers for missed services, routine health maintenance, and chronic disease management. A goal of this position is to support the clinical care teams in the management of Choptank patients to ensure they receive standard of care set forth by our overseeing regulatory bodies. This will ultimately improve health care outcomes and reduce hospital utilization. Activities may include providing education about recommended follow-up; scheduling appointments; communicating between patients and managed care organizations (MCOs); assisting patients in finding community resources to ensure preventative care needs are met.

Education and Experience:

  • High School diploma or equivalent required.
  • College degree in Health Sciences or a related field preferred
  • Two years of customer service experience
  • Two years’ experience in a medical office environment

Duties/Responsibilities:

  • Utilize patient lists generated by MCOs and other payers to make new patient appointments.
  • Assist with processes to ensure all new MCO patients have access to care within 90 days of enrollment.
  • Determine if assigned patients are actively engaged with another medical home and ensure this information is reported back to the MCO.
  • Use data from multiple resources such as EHR, MCO, and others to assist in patient engagement.
  • Collaborate with Care Coordinator(s), Nurse Navigator, Dental Case Manager, Patient Access Coordinator and Community Support Specialist(s) to use patient lists and contact patients and schedule them for appointments for routine and chronic disease visits with providers to close gaps in care.
  • Use IT platforms (i.e., CipherHealth, Solution Reach or EHR) for disease management and patient education/outreach.
  • Act as point person for MCO initiatives including Value Based Purchasing (VBP) and Provider Network Optimization (PNO) projects.
  • Regular, reliable attendance is a requirement of this job.
