Who are we?

AHL Healthcare Group, Inc. is located throughout Minnesota where we operate Community Residential Settings (CRS) homes for adults with developmental disabilities or mental illness. AHL- At Home Living has something for everyone, from country living to the suburbs Our Corporate office is in Duluth where we manage 12 CRS homes, 16 homes in our Metro office which is situated in Fridley, a suburb of the Twin Cities and our Metro West division, based in Cokato and its surrounding areas, recently expanded from 9 to 12 CRS homes.

However, it all started with 1 home back in 2000 that our founder opened and was the primary live-in caregiver. He discovered his passion-caring for people with disabilities A core foundation that we take pride in is leading by example, teamwork and providing a place called "HOME" for our clients and staff

Job description for a Float Program Coordinator (PC)? Thought you'd never ask

A Float Program Coordinator (PC) functions as an advocate, supporter, educator, and caregiver to individuals with disabilities. We believe in our clients and their right to live in the least-restrictive environment. We also ensure that the homes they live in allow them to lead fulfilled lives while remaining safe and well-cared-for.

Provide oversight to the program, monitoring that all company policies and procedures are being followed including day to day operations such as scheduling and when filling in at program, menu, activity level, safety, cleanliness, etc.

When filling in at a program, assure safety, health, and proper supervision of residents according to individual abuse prevention plans and program abuse prevention plans.

Plan and ensure that each resident's personal needs are met through the availability of adequate services, money, supplies, and shopping opportunities on an on-going basis.

When filling in at a program, keep the DOS updated with changes, appointments and needs within the program.

Demonstrate knowledge and assure all staff understand the Resident Rights and Vulnerable Adult policy, and ensure these are being followed.

Maintain professional contact with resident's case managers, guardians, doctors, other service providers and extended support teams.

Fairly distribute time amongst staff (working yourself if no replacements are available and when needed.)

Develop weekly staff schedule for Float DSP's, making on going scheduling changes in real time as needed and notify float staff of any changes.

Assist staff and residents with troubleshooting unexpected issues that arise (ie. client behavior, medical needs, incidents, staffing issues, etc.)

Driving to/from required locations as expected of the position with personal vehicle.

Tracking own mileage log for mileage reimbursement. Mileage reimbursement is not allowed for use of personal vehicle to locations expected of the position. (Supervisor meetings, dropping off on-call book/phone, etc.).

Maintain confidentiality at all times.

You would oversee 8 of our Adult Foster Care homes in Afton, Woodbury, Coon Rapids, New Hope, Maple Grove and 3 in Brooklyn Park

Hours of Position: Flexible based on house schedule

Compensation: $21.00 per hour

Requirements:

A baccalaureate degree in a field related to human services, and 1 year FT work experience *OR

An associate degree in a field related to human services, and 2 years FT work experience *OR

A diploma in a field related to human services from an accredited post-secondary institution, and 3 years FT work experience *OR

A minimum of 50 hours of education and training related to human services and disabilities AND 4 years FT work experience *under the supervision of a staff person who meets the qualification identified in 1-3 above.

(*work experience providing direct care services to persons with disabilities or persons age 65 and older)

Effective organization, ability to manage time and meet deadlines is necessary. Reliability, dependability, and flexibility are necessary for success in this position
