FLSA Status: Non-Exempt                                                                 OSHA Exposure Category: II



Position Summary:
Person in this position will be responsible for the case management in the Grants Per Diem Case
Management program. Essential Job Functions include those listed below.
Essential Job Functions:
• Assist Veterans with G2G program house with intakes, referrals and screenings.
• Coordination of services among multiple agencies.
• Attending scheduled meetings within the community.
• Attend and graduate f rom a certif ied peer support training program.
• Travel to community-based providers.
• Ability to provide independent life skills training, educational and coordination.
• Embrace and embody the mission, vision, guiding principles, vision, and goals of Community
Outreach Inc. and perform any other duties as assigned.


Essential Qualifications:
• Excellent communication skills.
• Good documentation skills.
• Familiarity with military culture.
• Be able to multi-task.


Education:
• HS Diploma or GED


Working Conditions:
• Work will primarily be conducted in a traditional office setting.
• Some travel within a two-county radius; and
• Some outdoor activities may be required.


Physical Demands:
• Occasional lifting of more than 10 pounds.
• Sitting, standing, walking, reaching are performed in the normal course of the position; and
• The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or
potential exposure may be required as a condition of employment. Appropriate personal protective
equipment will be readily available to every employee engaged in Category II tasks
