Job Details

Nurse Coordinator describes a variety of specialized nursing positions that provide assistance in planning, coordinating, and delivering patient care for selected patient populations across the continuum of care using the nursing process, ensuring compliance with physician orders and established policies and procedures. Nurse Coordinators provide coordination and care of patients requiring assistance and support in accessing and managing their health care needs (e.g., cardiac and cancer patients). They collaborate with other health care providers to ensure continuity of care and coordination of a wide variety of patient services. They will use performance improvement methodology to design data collection systems / processes and identify opportunities to improve care / service for patients. Responsibilities include coordinating patient contact, patient education, and preparing patients/families for procedures/tests in collaboration with health care team; collaborating with others to ensure continuity of care and coordination of a wide variety of patients services; identifying opportunities to improve patient care services; assisting clinic management to identify and develop operational needs for the clinic and to improve patient care services; and they may provide direct patient care.

Duties and Responsibilities

  • Collaborates with inpatient and other Clinic staff to develop informational/educational materials appropriate for the patient population. Develops, plans, and evaluates workshops, staff training/orientation and conferences. Assesses learning needs of patient populations and develops curriculum accordingly. Participates in formal and informal evaluation of staff competency.
  • Communicates with patients and their relatives to explain diagnostic and therapeutic measures related to condition, provide instruction, and promote participation in patient care.
  • Coordinates activities of nursing staff with other personnel and services engaged in caring for patients. Schedules physicians to ensure efficient use of time. Develops and implements processes / procedures to promote effective and efficient use of resources. Assists with implementation of new programs and assists with efforts to promote the service to other health care professionals and to the public.
  • Coordinates and participates in the delivery of safe, competent, compassionate age and specialty appropriate nursing care to a specified patient population. Serves as a liaison between groups of providers and/or care settings to ensure continuity of patient care. Facilitates access and communication when multiple services / agencies are involved. Collaborates with case managers and others to provide patient care. Serves as a consultant to nursing staff, medical staff, and other health care providers in care of the specified patient population.
  • Coordinates operations, develops and implements processes /procedures to promote effective and efficient use of resources. Assists in implementation of new programs and assists with efforts to promote the service to other health care professionals and to the public.
  • Demonstrates leadership by inspiring others to achieve their best and acknowledging the positive contributions of staff in the manner they wish to receive it. Promotes team building.
  • Develops project plans including major tasks, resources and time lines. Revises project plan when required and documents progress of project. Provides pertinent information on projects to key stakeholders and others.
  • Establishes/interprets Standards of Care and protocols. Applies organization's methodology to Performance Improvement projects. Utilizes results of quality assurance and research to identify opportunities for performance improvement. Keeps staff informed about Clinic Performance Improvement and Service Improvement efforts.
  • Identifies and defines problem situations considering all perspectives in terms of scope and consequences. Develops ideas and evaluates solutions to identified problems.
  • Interprets and enforces Infection Prevention and Safety policies. Reports unsafe conditions immediately.
  • Manages information utilizing appropriate manual or electronic systems. Maintains confidentiality and documents clinical information accurately and completely and within the specified time. Designs system for data collection to be used for clinical or operations performance measurements and/or research projects. Prepare for patient visit by collecting test results, old records and other reports and provides telephone triage for patient calls.
  • Maximizes productivity by focusing on goals, purpose and priorities. Completes written reports and projects within appropriate time frames. Reviews tasks, sets priorities, and revises plans for effective time management.


Requirements

Education Qualifications

  • Bachelor's Degree in Nursing from an accredited college or university

Experience

  • Three (3) years of progressively responsible and directly related work experience

Licenses and Certifications

  • RN - Registered Nurse
  • BLS - Basic Life Support
