What will you be doing:


We are looking for an experienced, talented, and compassionate Community Liaison for a weekend position based in the South Shore area.

The Community Liaison reports to the Director of Business Development and is responsible for new referrals to the Agency. You will serve as the primary liaison to prospective patients and designated health care providers (assisted livings, case managers, social workers, discharge planners) as well as insurance case management professionals and community groups to ensure safe and appropriate care for all new patients. The Community Liaison is a liaison to South Shore Hospital.

What we’re looking for in you:

  • BS in public health, health education or related field preferred
  • Previous experience in health care required
  • 2-5 years of experience in community health preferred.
  • Assisted Living experience preferred
  • Hospice Experience preferred
  • Excellent communication skills, oral and written, and the ability to communicate effectively and courteously.
  • Proficient clinical assessment and case management skills
  • Strong knowledge of Medicare/Medicaid and third-party insurers required
  • Able to problem-solve, trouble shoot quickly and efficiently
  • Able to review, analyze and trend clinical referral data.
  • Computer and keyboard skills necessary
  • Ability to work weekends
