Director of Social Work - Full Time - Days

As the Social Worker Director, you will play a crucial role in shaping the future of our Social Worker Department. You will oversee the development, implementation, and continuous evaluation of our programs aimed at helping residents achieve their highest level of well-being. Your responsibilities will include identifying and addressing the need for medically-related social services and ensuring that these services are delivered in strict compliance with state and federal regulations. Roles and Responsibilities:

  • The Social Worker Director will oversee and delegate the social work aspects of comprehensive assessments and care plans, ensuring goals and interventions are tailored to each resident's needs and preferences, and comply with Federal and State regulations, including promoting individualized, non-pharmacological care approaches.
  • Directing the Social Worker department includes overseeing preadmission Worker for inbound residents, to include participation in interdisciplinary evaluation of residents needs for institutional care and completion of the PASARR Level I.
  • Manage safe community transitions for residents by coordinating discharge planning and arranging follow-up care, while also helping residents and their families access financial, legal, and community resources.
  • Ensure that social worker actions are properly documented in medical records and that documentation adheres to legal, ethical, and professional standards.
  • Responsible for overseeing the establishment of departmental QA procedures and modification of those procedures where appropriate.
  • Manage Advance Care Planning for new residents, ensure regular review of Advance Directives, keep staff informed about residents' end-of-life wishes, and educate residents and their families about healthcare options and implications.
  • Assist residents in voicing and obtaining resolution to grievances. The Director will review complaints and grievances made by the resident and make a written report indicating what action(s) were taken to resolve the complaint or grievance. The Director will also gather grievance reports from other Social Worker staff and ensure follow-through and resolution has been completed as per facility policy.
  • Track grievance trends, report findings as part of the facility's QAPI program, and participate in policy development to enhance the quality of care for residents.
  • Ensure staff are aware of Resident's Rights and uphold residents' dignity, advocates for residents' rights, and leads investigations into allegations of abuse, neglect, or exploitation, reporting to the Administrator and relevant State agencies.
  • The Social Worker Director will ensure that residents who display mental illness, or psychosocial difficulties such as coping with grief and loss, will have access to appropriate treatment and resources.

Required Skills and Experience:

  • Bachelor's degree in social work or another human Worker field to include, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology.

Preferred Skills and Experience:

  • One year of supervised social work experience in a health care setting working directly with individuals.

  • Shift: (Mon-Fri) between the hours of 8:00 AM - 6:00 PM

Pay Range Disclosure

  • $70,616 to $97,614.40 per hour
