Job Summary: The Behavioral Health Care Coordination Services Manager helps with the overall development and coordination of this growing service line. This position works closely with medical, behavioral health services and quality and compliance leadership, contracted Behavioral and Mental Health services staff and leadership and, at times, works directly with patients who are seeking behavioral health services at VGMHC.

Essential Duties and Responsibilities:
• Manage the overall development and coordination of Behavioral Health Care Coordination Services including the process for internal Behavioral Health referrals, external specialty mental health and addictions services referrals and internal Medication-Assisted Therapy (MAT) services.
• Ensure ongoing quality of the Behavioral Health Care Coordination and referral processes, noticing themes in barriers to patient access to care.
• Work with related staff to problem-solve and improve the overall quality of care and access to care.
• Supervise Behavioral Health Care Coordinators at VGMHC sites. Provide regular training and ongoing individual and group supervision for these staff members.
• Ensure compliance with federal, state and local regulations related to behavioral health and addictions services, participate in internal audits and serve as a point-person for external auditors related to MAT. Work alongside the organization’s Quality Department and staff to coordinate ongoing compliance and quality standards for MAT services.
• Ensure the maintenance of records and reports, including credentialing and record keeping for all providers with a DEAx waiver and regular MAT patient panel reports.
• Work alongside key stakeholders to develop and evaluate clinical outcomes, continuously measuring, assessing and improving the quality of Behavioral Health Care Coordination and MAT services.
• Oversee and contribute to the expansion of Behavioral Health Care Coordination and MAT services across primary care sites, sharing best practices, policies and procedures as well as teaching and training to employees on MAT services. Develop relationships with key partners for the facilitation of Behavioral Health and MAT referrals and networks.
• Contribute to ongoing learning and education related to Behavioral Health Care Coordination and MAT services to ensure development best practices and that all services are delivered through a trauma-informed lens.
• Participate in opportunities to influence local, state and federal policy-making processes related to MAT and addictions services in primary care.
• Work with contracted Behavioral and Mental Health services staff and leadership, exploring and ensuring best practices in working with patients in Behavioral Health Care Coordination and MAT services as well as the process for facilitating and monitoring external mental health and/or substance use disorder treatment referrals.
• Coordinate with VGMHC providers who have a DEAx waiver, including those with subject matter expertise, to ensure program development occurs in alignment with clinical best practices and our organizational mission statement.
• Develop, monitor and provide oversight for Behavioral Health Care Coordination and MAT referrals, eligibility and patient caseload tracking systems as well as other external mental health and substance use treatment referrals, ensuring a closed-loop system and quality coordination of care.
• Coordinate and meet regularly with community stakeholders to develop and maintain referring relationships, participate in ongoing learning and educational opportunities and maintain awareness of key resources and supports in the community related to Behavioral Health Care Coordination and MAT services.
• Perform other duties as assigned.

HIPAA Requirements:
The Behavioral Health Care Coordination Services Manager will have access to PHI during the course of his/her work activities. Applying the minimum necessary standard of HIPAA, the designated record sets to which this position may have access include all sections of the medical record, patient demographic information in the practice management system, etc. This position is required to read the content of these records only to the extent needed to accomplish the assigned tasks.

• Proficiency in English and Spanish both written and spoken languages and experience within the Latino community.
• Valid driver’s license, reliable transportation, safe driving record and insurance coverage.

Education and Experience Preferred:
• Bachelor of Arts/Bachelor of Science degree in behavioral sciences field or at least four years similar work experience in healthcare or behavioral health setting.
• Experience supervising and training staff.
• Experience in program management and program development within a non-profit and/or healthcare environment.
• Experience providing direct services, particularly related to coordination of care between healthcare and behavioral health and/or addictions services providers.
