JOB SUMMARY

The Assertive Community Treatment Program provides intensive and all-inclusive case management and outpatient/rehabilitation services to homeless mentally ill individuals and incorporates a supported housing component. Incumbent will serve as Housing Specialist for Mental Health Integrated Services and Supported Housing with Assertive Community Treatment Team. The range of authority and decisions making for this position is limited to and consistent with MHS Policy for Housing Specialist staff.

RESPONSIBILITIES

  • Locates housing opportunities in defined areas of the County that comprise the housing continuum including short-term, transitional, and permanent options.
  • Coordinates and oversees various kinds of oversight agreements, including master leasing, scattered site leasing, and sponsor-based leasing.
  • Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences.
  • Updates team on a regular basis about housing alternatives and changes in availability.
  • Conducts extensive liaison with professional members of the housing industry as well as homeless and housing providers.
  • Provides information about the program to community agencies connected to the mental health services system to educate them about the program.
  • Assists clients in obtaining necessary documents for housing placement.
  • Document client contact using program electronic medical record.
  • Displays regular and reliable attendance. Reports to and departs from work on time, as scheduled, and accurately reflects all time on timecard.
  • Successfully functions as a team member.
  • Provide clinical interventions as it relates to housing needs.
  • Follows administrative chain of command.
  • Other relevant duties as assigned.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

QUALIFICATIONS

  • A.A. in social services field and three years’ experience providing housing-related social services to clients; OR
  • Bachelor’s degree and two years’ experience providing housing-related social services to clients.
  • Bilingual in Spanish, Vietnamese, Tagalog, or Arabic is preferred if the position serves that population.
  • Resourcefulness, initiative, and the ability to work with minimal direction and supervision.
  • Skills necessary to provide effective counseling, therapy and clinical services, and the ability to successfully work as a member of a treatment team.
  • Utilizes logical problem-solving techniques both independently and in collaboration with workers in dealing with client needs.
  • Proficient user of Microsoft Office Suite and EHR software.
  • Ability to problem solve and react appropriately to emergency situations.
  • Ability to work independently and effectively collaborate with a team.
