Job DescriptionJob Description

Essential Duties
• Assesses physical, environmental/safety needs of patients.
• Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Care.
• Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes for patients.
• Provides in-patient facility nursing care according to the plan of care.
• Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
• Accurately and timely documents nursing assessments, interventions, and updates to Plan of Care according to the nursing standards of this organization.
• Supervises LPN’s and nurse aides and documents supervision according to the organizational policy.

Requirements
• Graduate of accredited school of nursing- ASN; BSN Preferred
• Current North Carolina Registered Nurse license
• One year of clinical nursing experience preferred
• Certification in Hospice and Palliative Care is desired
• Ability to lift, turn and transfer patients; minimum lifting requirement 40 pounds
