We are seeking a skilled and compassionate Hospice Nurse Case Manager to join our team at Hospice of the Piedmont.
Key Responsibilities:- Develop and implement individualized plans of care for patients in accordance with their expressed wishes and in collaboration with the interdisciplinary care team.
- Provide safe, competent, and compassionate hospice nursing care to patients in their homes, assisted living facilities, or other settings.
- Coordinate care with other healthcare providers, family members, and caregivers to ensure seamless transitions and optimal patient outcomes.
- Accurately and timely document patient assessments, interventions, and updates to the plan of care.
- Supervise and delegate tasks to LPNs and nurse aides as needed.
- Current RN licensure in the state of practice.
- Experience in hospice or palliative care nursing.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic medical records and other relevant software systems.
