Reports to: Homeless Prevention Program Manager
OVERVIEW: Founded in 1979 in direct response to rising rates of hunger and homelessness in North San Diego County, Interfaith Community Services works to break the cycle of poverty and homelessness by Helping People Help Themselves. Interfaith is the most comprehensive social services agency in North County, providing both immediate safety net services and long-term, life-changing programs designed to meet the holistic needs of people in crisis. Together, in partnership with 250+ diverse faith communities, more than 650 recurrent volunteers, and a growing staff of over 200, Interfaith provides a safe haven and pathway towards self-sufficiency for more than 17,000 community members annually.
WHAT YOU WILL DO: This position will work with families and individuals experiencing or at risk of experiencing homelessness. The case manager will provide a variety of office and field activities as needed, to support stabilization of individuals and families. The case manager will ensure that programs, providers and identified housing are following all applicable Federal and local housing regulations, laws and standards. The Case Manager will provide supportive services and resources, manage financial assistance funds and advocate for clients to landlords and property management companies.
• Provides direct services to clients including diversion, Individual Service Plan (ISP), assessments, crisis intervention, advocacy, and case management
• Utilizes Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness
• Administers supportive services per agency and contract guidelines
• Identifies resources and advocates for clients’ needs
• Ability to work with and collaborate with volunteers, interns and other agency personnel.
• Assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency; develop an effective, timely referral network of services and support that assist with housing retention in order to ensure ongoing direction and support as needed
• Assist in development of and encourage adherence to a personal budget through proactive housing and budget counseling sessions; provide budget counseling and education to assist clients in establishing payment plans for bills and past debts and to assist clients in obtaining and maintaining their housing
• Identify participant strengths and barriers to stability and assist participants to determine housing and service needs. Resolve or mitigate tenant screening barriers as applicable.
• Provide guidance and support to help clients identify their own strengths, set achievable goals and follow through consistently to improve and achieve self-sufficiency
• Provide pro-active follow-up appointments to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, listening, problem solving, and identification of resources to assist with reintegration of participants in the community
• Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, program staff)
• Serve as an ongoing liaison between property managers and participants; provide mediation and advocacy with landlords on the client’s behalf to develop a workable plan to obtain and/or maintain housing
• Complete documentation and enter data into client database in an accurate and timely manner
• Collect and report program data, including HMIS (Clarity) reporting and funders’ required data.
• Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs
• Apply knowledge of residential lease contracts to educate clients of their rights and responsibilities
• Participate in supervision, staff meetings, and trainings as assigned
• Perform additional duties as assigned
Requirements:EXPERIENCE / EDUCATION
• BA degree in social work or related field preferred; equivalent combination of work experience and education may be considered
• 1+ years of experience working in a housing, social services or nonprofit setting providing goal-oriented case management/motivational interviewing preferred
• Experience working with homeless and/or vulnerable populations preferred
• Bilingual (English/Spanish) preferred
