Purpose

Case Managers provide a range of services across multiple programs which include Foster Care, Independent Living, Family Preservation, Reunification, and Diversion. All programs have the goal of restoring hope in individuals and families. Services, responsibilities, and clients vary depending on the Case Manager’s program(s). Case Manager will be responsible for all casework and therapeutic interventions.

Summary

The Case Worker meets with the family to assess the capability for program services to maintain safety of the child and family, as well as the family’s willingness to participate with program services. The Case Worker contacts the Cabinet to communicate the safety of the child, the family’s acceptance of services, and to arrange the ongoing communication and contact between Bellewood & Brooklawn and the referring DCBS worker throughout the duration of services to the family. The Case Worker consults with the DCBS referring worker, at least weekly, throughout the duration of services and more often if necessary.


Essential Job Functions

  1. Provides in home and out of home case management services to a caseload determined by their supervisor and not to exceed regulatory limits.
  2. Ensures coordination, delivery, and documentation of appropriate services to clients, working collaboratively with outside providers and other agency staff.
  3. Builds rapport and positive relationships with youth and their families.
  4. Conducts and documents face to face and/or indirect contacts with assigned youth and families at minimum, weekly, according to program guidelines.
  5. Works closely with youth and their caregivers, therapists, schools, and other team members to assess needs and safety and achieve best outcomes for clients
  6. Consults with the referring DCBS worker routinely throughout the duration of services
  7. Maintains professional relationships with youth and their families, social workers, school staff, therapists, foster parents, caregivers, and other community and team members.
  8. Completes program specific assessments with youth and families
  9. Participates in the development of plans of service, prevention plans, and aftercare plans together with youth and their families and therapists when applicable.
  10. Assists those served to identify and modify negative or high-risk behavioral patterns
  11. Helps those served practice and celebrate small changes.
  12. Connects families to community resources to enhance family functioning.
  13. Is available according to program specific “On Call” procedures.
  14. Responds to client behavioral and emotional crises and offers consultation, coaching, and/or in person intervention as needed.
  15. Develops, prior to case closure, a transition/aftercare plan and conducts follow ups as required.
  16. Completes documentation of services provided and progress made in accordance with program specific regulatory requirements.
  17. Completed intake and discharge documentation as required by program.


Starting Pay

$18.72 per hour.

Requirements:

Education and/or Experience

Bachelor's degree (B. A.) from four-year College or university in Social Work, Psychology, or a closely related field and at least one-year, full-time employment working with multi-problem families and familiarity with crisis intervention, developing behaviorally specific goals and the ability to teach skills.
