SUMMARY:
The Residential Nurse works under the direction of the Residential Director, and is responsible for planning and coordination of medical services for adults who receive residential support(s) from FRA. The Nurse is responsible for expediting activities with medical personnel and vendors, discharge planning, level of care assessments and renewals, monitoring participant placement, training assisted living specialists, and working as a team member with ancillary agencies. The Residential Nurse assists in the development and updating of individual service plans including Nursing Oversight and Care Management (NOCM) plans. The Residential Nurse is also expected to perform home visits, skills training, and direct care as needed.
TASKS, DUTIES AND RESPONSIBILITIES:
- Monitors the safety, security, care and welfare of participants in the home. Provides direct assistance in identified problem areas.
- Conducts face-to-face visits and telephone contacts with assigned participants and/or guardians at least quarterly; more frequently as needed.
- Provides training to direct service staff and Residential Site Supervisors.
- Tracks critical deadlines, i.e. medical and specialty clinic appointments, and immunizations.
- Participates in the development and implementation of Support Plans (SP’s).
- Maintains close communication and collaboration with the Senior and Disabilities Services staff, hospital care coordinators and discharge planners, and others to advocate for needed services.
- Communicates with guardians regarding individuals’ medical needs including care plans, medical appointments, and necessary medical supplies and equipment.
- Trains and updates residential staff on relevant medical procedures and processes including medication administration.
- Manages NOCM plans including training checklists.
- Coordinates medication management including pick-ups, refills, proper storage and disposal and quality assurance of the Medication Administration Records (MAR).
- Completes Critical Incident Reports (CIR’s) in accordance with agency procedures, as well as, state and federal requirements.
- Maintains participant records in accordance with State of Alaska standards of licensing for Nurses.
- Inputs participant information and case notes into electronic medical record, in accordance with agency procedures, as well as, state and federal requirements.
- Attends training seminars, in-service sessions, and staff meetings.
- Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
- Provides other support services and duties as necessary.
MINIMUM EDUCATION AND EXPERIENCE:
- State of Alaska Registered Nurse.
- Two years’ experience with adults who experience developmental disabilities preferred.
- Experience with Public Health nursing preferred.
- Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Demonstrates knowledge of and ability to apply principles of normalization and least restrictive environment, as well as the philosophy of the organization. Ability to train and provide technical assistance to direct care staff. Ability to problem solve and make decisions. Ability to effectively communicate with participants, families, other staff, personnel of other agencies and the general public. Ability to write plans of care and conduct assessments. Ability to work a flexible schedule. Must have reliable transportation and maintain a good driving record in order to conduct on-site visits. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications.
LICENSES/CERTIFICATIONS REQUIRED:
Alaska Registered Nurse License
Alaska Driver’s License
Certification in Adult, Child-Infant CPR
Certification in First Aid
**Safety Care Certification
**QBC Certification
Ability to pass a background check
**Must be able to obtain these certifications within 3 months of hire through FRA’s paid training courses.
IMMUNIZATION REQUIRED:
Annual PPD Test
Hepatitis B immunization (or) Waiver
WORKING CONDITIONS:
Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects and assist participants.
PHYSICAL ACTIVITIES:
Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
Talking: Expressing or exchanging ideas by means of the spoken word. Those activities which demand detailed or important instructions spoken to other workers accurately, loudly or quickly.
Repetitive Motions: Substantial movements (motions) of the wrist, hands, and/or fingers, especially as related to typing notes and reports.
Driving: Minimum standards required by State Law (including license).

Occupational exposure to bloodborne pathogens.
