Overview

Are you a clinician with looking improve your skills in serving and advocating for individuals with mental illness? Are you interested in contributing to social justice for people impacted by the criminal legal system? If so, we need you on our Brooklyn-based Nathaniel Community Success (NCS) team at CASES NCS is a new alternative-to-incarceration (ATI) program that serves individuals of all genders, aged 16 and older, with mental illness (including substance use problems); this program operates in Manhattan and Brooklyn and will leverage the expertise of CASES in jail and prison alternatives and mental health treatment, including operation of an outpatient clinic and care coordination services. In doing so, NCS will directly help reduce the utilization of jails and prisons.

We are seeking a Case Coordinator for our Brooklyn NCS team, who will join a multidisciplinary team comprised of a Team Leader, Clinical Forensic Case Coordinator, Substance Use Specialist, Peer Specialist, Clinical Criminal Legal Specialist, and Program Assistant. The NCS team provides individualized clinical case management; linkage to services for mental health (including substance use problems) and medical treatment; and court-ordered supervision services. Each participant’s plan is individualized, responding to specific needs and risks to help each participant achieve and maintain recovery and reduce criminal recidivism. The Forensic Clinical Case Coordinator will be responsible for providing evidenced-based, clinically-oriented services to address certain risk factors, such as decision-making and self-regulation skills. CASES is looking for an individual who is committed to social justice, evidence-based practices, effective violence risk management, and the principles of recovery-oriented and trauma-informed services.

We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This job requires work in our offices, court houses and the community.

Responsibilities

  • Collaborate with NCS Brooklyn colleagues to collectively manage the Brooklyn team caseload.
  • Provide in-person services in the program office in Brooklyn, as well as in the community, with the location of service delivery determined by participant need.
  • Conduct onsite program groups according to training and competence to engage participants in onsite services.
  • Provide intensive case management services by communicating with community treatment providers, conducting site visits, and attending case conferences to engage and support participants in community treatment.
  • Assess participants’ immediate needs, legal circumstances, and engagement needs.
  • Use Motivational Interviewing to identify risk factors for criminal recidivism, inform participant of risk factors, and identify community resources to support participant to reduce dynamic risk factors and address needs.
  • Act as advocate and liaison for participants in securing access to community services, housing, income support, and natural supports such as self-help.
  • Complete charting using database following program standards and timelines.
  • Involve family, significant others, and participant-identified collateral contacts in service provision.
  • Collaborate with team members to provide coverage for after-hours reporting schedule to support participants engaged in employment, vocational training, and education services.
  • Provide culturally competent services in accordance with CASES policies and practice.
  • Develop and maintain strategic alliances with community providers.
  • Required to be in court, office or field 5 days per week.
  • Perform other related duties as assigned by Team Leader.

Qualifications

  • Bachelor’s Degree in Social work, Criminal Justice or other related field is preferred
  • Minimum of two years of experience working in human services with individuals with mental illness, substance use disorders, co-occurring mental health and substance use disorders and involvement in the criminal justice system.
  • Experience understanding various models and theories pertaining to treatment of mental illness and substance use.
  • Willingness to learn and adopt evidence-based principles to reduce recidivism risks.
  • At least one year of experience conducting group sessions using evidence-based practice.
  • Extensive knowledge of community treatment, support services, and resources.
  • Highly organized and self-motivated.
  • Proficient in computers.
  • Fluency in Spanish preferred.
