THE INS AND OUTS OF BECOMING OUR CRITICAL CARE COORDINATOR
YOUR ROLE:
As a Critical Care Coordinator, your day begins by evaluating and interpreting referral information and reaching out to families within seven days. You complete comprehensive assessments, considering the child's strengths and cultural background, and use IM+CANS to create individualized service plans. You update these plans as needed and ensure all documentation is uploaded to the HFS IM+CANS Provider Portal. Collaborating with families, you develop Crisis Prevention and Safety plans, identify natural and professional supports, and facilitate Child and Family team meetings every 60 days or post-crisis. You engage with families regularly, including monthly home visits recommending services, coordinate with MCO Case Managers, and ensure youth's needs are met through partnerships with treatment providers and schools. Maintaining accurate case records, you manage a caseload of up to 25 families, delivering strength-based, culturally competent, and trauma-informed services daily.
OUR REQUIREMENTS:
- Bachelor's degree in counseling and guidance, rehabilitation, social work, education, psychology, or related human service field, along with two years of supervised experience
- Ability and willingness to actively participate in a fast-paced, multidisciplinary team environment
- Knowledge of community programs, services, evidence-based practices, and a variety of treatment modalities
- Possess and maintain a valid driver's license and provide proof of insurance
- Strong organization and time management skills
- Excellent interpersonal and written communication skills
- Must possess the knowledge, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines
