The Wraparound Service Coordinator is a critical role within Hoyleton Youth and Family Services, responsible for providing comprehensive support to youth and families enrolled in the Wraparound Program. This position requires a strong understanding of the program's philosophy and a commitment to delivering high-quality services that meet the unique needs of each family.
Key Responsibilities- Evaluation and Assessment: Conduct thorough evaluations of referral information and complete strength-based assessments with youth and families to identify their needs and develop effective care plans.
- Service Coordination: Develop, document, and maintain individualized Wraparound Plans in collaboration with youth, families, and referring agencies, ensuring that services are tailored to meet the specific needs of each family.
- Community Engagement: Establish and maintain relationships with community resources, treatment providers, and schools to ensure that youth and families receive comprehensive support and services.
- Case Management: Oversee the transition of youth and families from the Wraparound Program to ongoing care, ensuring a smooth and successful transition.
- Documentation and Reporting: Maintain accurate and timely records of case activities, progress, and outcomes, in accordance with program requirements.
- Education and Experience: Bachelor's degree in a related field, such as counseling, social work, or education, along with three years of supervised experience in a related field.
- Skills and Abilities: Strong organizational and time management skills, excellent interpersonal and written communication skills, and the ability to work effectively in a fast-paced, multidisciplinary team environment.
- Certifications and Training: Possession of a valid driver's license and proof of insurance, as well as completion of CARE and TCI training programs.
