Job Summary

We are seeking a highly skilled Patient Navigator to join our team at CrossWinds Counseling & Wellness. As a Patient Navigator, you will play a critical role in providing care coordination services and assisting clients through our Same Day Access or scheduled intake process.

Key Responsibilities
  • Provide care coordination services to clients, ensuring they receive the necessary support and resources to achieve their health goals.
  • Assist clients in navigating our healthcare system, connecting them with the right programs and services to meet their needs.
  • Collaborate with internal and external providers to increase the frequency of desired outcomes and improve the quality of care provided.
  • Build effective relationships with clients, families, and community partners, emphasizing client strengths, choice, and empowerment.
  • Ensure that key client information is communicated to direct service providers and support staff prior to and after appointments.
  • Facilitate collateral communication between clients, support staff, direct care providers, and the community to assist clients in meeting all health-related goals.
  • Develop and maintain positive working relationships with all resources within and outside the agency.
  • Enter CMHC & CCBHC data into all required systems within assigned timelines and requirements of the CCBHC model.
  • Document services provided and client responses in a timely, current, and accurate manner.
  • Conduct initial in-person client intake interviews, ensuring all necessary demographic, financial, insurance, and basic health and family history data is obtained.
  • Provide quality customer service to clients, staff, and community partners.
Requirements
  • Bachelor's Degree or equivalent combination of education and experience in the Human Services field.
  • Ability to work with persons with serious emotional disturbance and/or severe and persistent mental illness.
  • Competency in conducting highly-organized and detail-oriented work.
  • Excellent customer service skills.
  • Ability to handle and respect confidential, sensitive information.
  • Prior computer experience in Microsoft Office programs and moderate to advanced computer fluency.
Work Environment

This is a full-time position, Monday through Friday, 8:00 am to 5:00 pm. The position does not require travel.

This job operates in a professional office setting, using standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
