The purpose of the Service Provider is to give youth and families the information and skills needed to function as part of

their community and independently access resources with minimal risk of institutionalization or other removal of the

youth from their home. Under the direct supervision of a Service Provider Supervisor, the Service Provider (SP) provides

behavioral and mental health care services and support to youth and families in need of extra support as determined by

the youth’s Care Manager (CM). The SP will provide individual, group, and family

support services in the least restrictive environment. The SP seeks to develop and nurture a professional therapeutic relationship with a population of

seriously emotionally disturbed, developmentally delayed, or medically fragile youth. The SP must also maintain

appropriate professional relationships with the youth, members of the youth’s Care Team, and fellow SPs to provide

optimal care and quality services.


Mentoring youth in developing positive peer and family relationships.

Organizing/introducing youth to age-appropriate social, recreational, and educational events and activities.

Assisting youth and family with managing behaviors/symptoms to improve functioning as a family.

Communicating and collaborating effectively with the youth’s Care Team (Caregiver, Care Manager, Supervisor,

other Service Providers, etc.).

Practicing the utmost discretion and safety practices with youth in their homes and communities.

Completing all paperwork (Service Summary Forms, Verification of Service Forms, Progress Notes, Service Plans,

etc.) promptly and professionally in compliance with program regulations and deadlines.

Adhering to the goal and objectives of the youth’s Service Plan within the limits of the approved frequency and

scope of services.

Attending team meetings regarding the youth’s care (i.e., Interdisciplinary Team meetings, Plan of Care

meetings, etc.).

Completing all mandatory training sessions before the start of services.

Attending Department and Supervision Meetings.

Performing other responsibilities as assigned.

Requirements:

CASELOAD REQUIREMENTS:

Per Diem Service Providers must provide a minimum of two (2) services in one (1) week. The number of clients

may vary based on the number of services each client is referred to receive. To maintain this minimum number

of services, the Service Provider will need to uphold the recommended frequency and accept the assignment of

new clients as necessary.

QUALIFICATIONS:

EDUCATION:

Bachelor’s degree

EXPERIENCE:

A minimum of three (3) years in the human services field required

REQUIRED SKILLS:

Proficient skills in Microsoft Office

Excellent oral and written communication skills

Cultural knowledge or sensitivity

Business acumen

Strong organizational skills and documentation skills

Strong reading comprehension skills

PREFERRED SKILLS:

Bilingual (Spanish-speaking)

TRANSPORTATION:

Must be willing and able to travel to different boroughs to provide face-to-face services.

BACKGROUND:

Section 424-A of the New York Social Services Law requires an authorized agency to inquire whether a candidate for

employment with child-caring responsibilities has been the subject of a child abuse and maltreatment report.
