Position Purpose

Oversees the overall operation of the Health Services Department in accordance with current federal, state, local regulations that govern Personal Care Homes, Assisted Living Centers, Independent Living Program and Community Access Group to ensure the highest degree of quality care. Ensures Annandale’s policies and procedures are followed and maintained at all times, full authority and responsibility for operation of the Private Home Care Provider. Collaborate and coordinate with administrators, and managers of other departments to ensure the smooth integration -and delivery of supports and services to all Villagers.

Required Qualifications

Graduate of an accredited 4-year college or university of nursing (preferred), and currently licensed as an RN in Georgia.


Bachelor’s degree in nursing or related healthcare field.

A minimum of five years demonstrated competence; knowledge of nursing theory and practice, policies, procedures and established standards of care and regulatory agencies.

Knowledge of personnel administration and managerial principles and functions.

Must be at least 21 years of age, have a valid Class C Georgia Driver’s License, proof of vehicle insurance, and meet company policy regarding MVR requirements.

Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.

Must undergo and pass a drug screening and screening for Tuberculosis.

Major Duties and Responsibilities

Provides leadership and oversight of all operations and performance activities related to Health Services at Annandale at Suwanee, Inc.

Assists the PCH/DP Administrator in overseeing and managing the proxy care program.

Responds to family concerns regarding Villager health and medication issues in a timely manner.

Responsible for managing healthcare aspects for all state licenses and contracts, including Healthcare Facility Regulation licenses and Department of Behavioral Health and Developmental disabilities contract.

Develops, maintains and updates written nursing policies and procedures for PCH, ALC and All-In. Facilitates the implementation process of same.

Responsible for appropriate follow-up on clinical incidents, including notifications to family, CPO and Compliance Director

Plans, develops, organizes, implements, evaluates, and supervises/oversees all Health Services staff, programs, and activities.

Is the primary liaison between Annandale’s selected physician group and pharmacy. Coordinates medication order, re-order, delivery, and verification processes.

Partners with the admissions team to be an integral part of the admission process. Duties include, but will not be limited to, assessing potential Villagers' health needs, collaborating with various members of the team to make admission related decisions, communicating with families, and other health related assessments and discussions regarding new Villagers.

Oversees individual client services in a person-centered manner in collaboration with individual’s medical provider

Monitors residents’ treatment and medication plans to ensure residents are receiving proper care; reviews nursing documentation, MAR’s, etc. to ensure compliance with regulations.

Ensures that health services documentation in the electronic record system is current including incident report follow up.

Assists in developing, implementing, and maintaining safety standards, written aseptic and isolation techniques, programs for monitoring communicable or infectious diseases, and procedures for reporting hazardous conditions or equipment.

Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the residents.

Employee may be required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
