POSITION PURPOSE/SUMMARY
Working as part of an interdisciplinary team and in consultation with other team members, is responsible for the supervision of and participation in the day-to-day direct care staffing and programming, addressing chronic care needs and incorporating a person-centered, individualized care model.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Oversees day-to-day operations of the Program including facility maintenance. Ensures that repairs and inspections are completed in a timely manner. Ensures adequate equipment and supplies to maintain the safety and cleanliness of the facility.
2. Hire, train, supervise and motivate competent direct care staff.
3. Fulfill duties of Activity Coordinator in their absence and sign off on activities scheduled, when needed.
4. Full implementation of Person-Centered Care (PCC) to ensure all staff are incorporating values and techniques of engagement to address personhood, program and physical setting. Evaluates PCC practices and monitors training needs.
5. Participates in the interdisciplinary team formulating Plans of Care for program participants.
6. Oversees all programming including individual and group activities designed to enhance participant’s sense of dignity, contribution, and self-worth.
7. Makes daily decisions on staffing needs, program structure and safety.
8. Oversees intern and volunteer responsibilities.
9. Stays current in adult day program recreations therapy trends.
10. Ensures participants cultural and ethnic traditions are valued and appropriately acknowledged.
11. Oversees the financial operations of the center. Monitors expenditures to stay within budget. Assists the Director of Community Programs and Yolo Hospice CFO with development of annual FTE and operations budget.
12. Participates in Quality Assessment Performance Improvement (QAPI) under the direction of the Chief of Quality, Compliance, Organizational Development. Ensure compliance with all applicable regulatory standards (accreditation, licensure).
13. Develops and implements policy and procedures. Ensures staff is continually educated in policies and are following standards.
14. Ensures completion and submission of incident/accident reports within deadlines.
15. Models a customer service attitude. Effectively communicates with staff, families and caregivers.
16. Promote the safety and welfare of participants at all times.
17. Regularly schedules department and staff 1:1 meetings to provide leadership, development and to encourage communication between all members of staff.
18. Attends and participates in appropriate in-service and department meetings.
19. Follows established emergency procedures.
20. Complies with policy to ensure patient and organization privacy and confidentiality at all times.
21. Complete CPR and first aid training, maintain certificates as required.
22. Performs other duties as assigned.
Requirements:JOB QUALIFICATIONS
1) B.A. in Psychology, Social Work, or other human service-related field preferred.
2) Must meet one of the following:
a. Have two years’ experience in a social, recreational or educational program within the last five years, one of which was full time
b. Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist or occupational therapist assistant or a qualified social worker or
c. Have completed 36 hours of training in a course designed for this position.
3) At least 2 years supervisory experience.
4) Experience working with adults and elderly preferred.
5) Must possess excellent communication skills (verbal and written) to effectively provide care, communicate with caregivers and family members and document in writing participant progress and activities.
6) Must be able to organize and lead recreational and social activities.
7) Proficient in Microsoft Office Outlook, Word and Excel. Familiarity with other systems including EMR a plus.
8) Strong attention to detail, excellent time management and organizational skills.
9) Must be able to perform effectively under stress and when confronted with critical or unexpected events.
10) Must be able to work independently.
11) Must be willing to undergo a medical screening, including TB test.
12) Must pass agency background check.
13) Must be at least 21 years of age.
PHYSICAL & OTHER JOB REQUIREMENTS
1. Considerable walking and standing.
2. Occasionally stoop, kneel, crouch, or crawl.
3. Ability to lift up to 35 pounds.
4. If driving on Yolo Hospice business, must have a valid California driver’s license and state-required automobile insurance.
