Area Agency on Aging and Community Action Agency for Northwest Indiana, founded in 1965, the CoAction Mission is to ensure the people who experience financial hardship find opportunities, resources, and respect. We do this by helping individuals flourish regardless of income, ability, or age. Serving residents in Lake, Porter, Jasper, Newton, Pulaski and Starke Counties.


FUNCTION:

Reporting to the HCBS Program Manager, the Service Coordinator works directly with professionals to coordinate service delivery to those in service. This role coordinates care plans and communication across agencies for individuals receiving services from multiple healthcare or social service providers. The Service Coordinator will work closely with healthcare professionals, social workers, and other community partners to ensure that individuals receive high-quality, coordinated care that meets their physical, mental, and social needs.

 

MAJOR DUTIES:

  • Cross-Agency Coordination: Develops and implements systems to keep the health and social service professionals connected to each other’s service. This includes communicating across agencies the goal achievement or barriers towards goals and ensuring interdisciplinary coordination occurs.
  • Facilitate Communication: Initiates written and verbal communications among community partners that address the participant’s physical, mental, and social needs.
  • Policy and Procedure Development: Develops and implements policies and procedures related to aging services, ensuring compliance with regulatory requirements.
  • Participant Engagement: Monitors and recommends participant engagement in the services provided. Helps with continuity of care when there is change in an interdisciplinary team member.
  • Quality Assurance: Develops and implements quality assurance programs to ensure that services are provided in a timely and effective manner.
  • Other duties as assigned by the Director of Aging Services.

 

MINIMUM QUALIFICATIONS:

  • Bachelor's degree in healthcare, social work, or related field preferred.
  • At least two years of experience in healthcare, social work, or related field
  • Strong communication and interpersonal skills in verbal and written format.
  • Strong organizational and problem-solving skills to allow independent work and decisiveness.
  • Ability to work collaboratively with staff, participants, families, and healthcare providers.
  • Familiarity with electronic medical records and other healthcare technologies.
  • Knowledge of relevant regulations, laws, and industry standards related to Medicaid, Healthcare, and Aging Services.
