The Safety Net Services Case Manager provides full and complete direct SN case management services to families, youth, and children for Differential Response (DR), Concrete Support Services (CSS), and Post-Reunification Services (PRS) countywide. The Case Manager is responsible for initial face-to-face contact with referred client/family, for evaluation, determinations, and document home visits for data entry and referral/follow-up to resources, create case/service Family Services Plans (FSP), assess client needs and in consultation with clients, make service referrals, provide case management until family/clients are stabilized, maintains active, open, and on-going communication with all service providers, referred families, Program Manager and CPS/HSA Social Workers, including and not limited to those duties outlined within the proposal and RFP. Weekly, Case Managers meet with the Program Manager to discuss cases, possible issues and continual case evaluation. Case Managers must be able to effectively communicate, verbally and in writing, with parents and children in English and Spanish (or in any other threshold language they have been hired to provide). They must maintain active and open communication with Child Protective Services and case carrying Social Worker. Attend and participate Team Decision Making (TDM) meetings as necessary. Develop a service plan with input from case carrying Social Worker and referred family.

Employment Type: Full-Time; Non-Exempt

Requirements: Background Check, Health Records/Immunizations, Valid Driver's License.

Qualifications: A Bachelor's degree with a major in psychology, sociology, or related field is highly preferred. In lieu of a bachelor's degree, Case Managers may meet staffing qualifications with 60 completed semester units with a major coursework in psychology, sociology, or a related field plus two-years of experience providing case management services to children and families.

Leave: El Concilio offers PTO (Personal Time-Off) benefits as well as 13 paid holidays per year to all permanent employees. Part-time employees' leave benefits are prorated.
