Position Summary:

The RN Clinical Coordinator supervises and/or provides direct patient care for the Adult Hospice Program. The position supervises all other members of the Interdisciplinary Group either directly or indirectly through their immediate manager. The incumbent must be effective in explaining the hospice concept of care to patients, families, physicians, resource and referral sources and other community members.

Requirements:

Essential Job Functions:

1. Follows an individualized Plan of Care established by the RN for each patient

2. Adheres to patient rights, abuse reporting and confidentiality policies

3. Fosters and maintains an effective working relationship between peers, professional staff, and other personnel

4. Understands/provides basic principles of palliative care/pain management within the Scope of Practice

5. Attends scheduled staff meetings and or in-service training

6. Adheres to the departmental dress code requirements related to personal grooming and attire

7. Participates in Performance Improvement activities as requested

8. Demonstrates reliability by: Making every attempt to report to work on time, providing proper notification for absence or tardiness, participating in required meetings

9. Demonstrates ability to problem-solve and make decisions by: Assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of command

10. Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency, willingness to adjust or revise work schedules as requested

11. Assumes responsibility for on-going personal development and continuing education

12. Stays current with all mandatory agency and regulatory requirements

13. Manages budgets, hire, and train staff and assist in developing and implementing policies and protocols

14. Completes assigned projects within specified time frames

15. Other duties, as assigned

Additional Functions:

1. Hires, counsels, administers progressive discipline assigned staff members as needed

2. Assists in creating and implementing policy/procedures

3. Participates and interacts in all complaint processes as directed

4. Ensures that all regulatory standards are met State, Federal, CHAPS and other regulatory bodies

5. Assists in developing effective plan of Correction in response to surveys as needed

6. Evaluates and conducts performance evaluations of staff

7. Implements a delegation of responsibility for the availability of clinical services on a twenty-four (24) hour basis

8. Manages clinical aspects of patient care

9. Participates in budgeting process as requested by the Director of Nursing

10. Assures patients are admitted under correct requirements for hospice service

11. Assures proper staffing and scheduling for patients

12. Supervises and delegates duties to multidisciplinary clinical personnel

13. Assists with coordination of Interdisciplinary Team Meetings

14. Coordinates staff meetings and/or in-service training as needed

15. Manages confidential information

16. Maintains regular and consistent attendance

Physical/Safety Requirements:

1. Must have the strength and endurance required for standing, walking, lifting, bending, and pushing on a consistent basis between 10 and 50 lbs.

2. Utilizes appropriate durable medical equipment and PPE as needed in a safe manner

3. Uses appropriate body mechanics as needed for personal use and safe patient care

4. Category 1 for potential exposure to blood/body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them. Protective measures should be used while engaged in Category 1 tasks with exposure potential.)

5. Follows the designated plan of action in the event of a fire or other emergency/disaster

6. While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stresses, and driving hazards

7. The individual may be required to drive to meetings with clients, patients’ homes, or health care professionals

8. The noise level in the work environment is usually minimal

Qualifications and Experience:

1. Must possess a valid RN license with the State of Nevada Board of Nursing

2. A minimum of 1 year of experience as a RN in a hospice/home health setting is required

3. CPR/First Aid certification is required

4. A minimum of one (1) year relevant hospice experience is required

5. Must successfully complete a background check pursuant to NRS 449.188 and NRS 179A.100, verification of previous employment, references, and an MVR (may include verification of education)

6. Must be able to read, speak, write, and understand the English language

7. Excellent verbal and written communication skills are required

8. Must be able to handle frequent interruptions

9. Must be able to cope with emotionally charged situations encompassing patients, families, and other caregivers

10. Must be available for prescheduled on-call hours, weekends, and holidays

11. Must be available to cover shifts as needed and call coverage as needed for direct patient care needs

12. Must be able to prioritize, order medications, DME, and supplies using the hospice approved formulary and supply list

13. Must have reliable transportation, a valid driver’s license, and current auto insurance
