Position Overview: The Program Coordinator is responsible for organizing services to ensure that patients receive optimal care and assistance. This role involves health education, outreach, and health coverage enrollment services for high-need families in underserved communities. As an integral member of the health education and outreach team, the Program Coordinator works closely with medical providers, clinic staff, patients, community members, volunteers, and other organizations to improve health outcomes.

Key Responsibilities:

  • Assist with planning, coordination, and tracking of programs and their activities.
  • Develop and monitor program workflows and practices.
  • Keep programs on schedule by monitoring deadlines and prioritizing tasks.
  • Participate as an integral part of the quality assurance and quality improvement team.
  • Coordinate participant outreach and caseloads according to program guidelines.
  • Liaise between the Health Education Department and the Finance or Billing Departments.
  • Coordinate interactions between staff, patients, administrators, and other program stakeholders.
  • Schedule and organize program-related meetings and events.
  • Assist with creating meeting agendas and compiling necessary paperwork for documentation.
  • Promote and facilitate health access through education, outreach, and enrollment activities.
  • Establish trusting relationships with at-risk families by initiating and maintaining regular client contact.
  • Serve as a trusted liaison for clinic staff, patients, and the community.
  • Maintain an up-to-date account of quality community resources and provide effective linkages to services.
  • Assist with group health education on various topics including chronic conditions, asthma, diabetes, obesity, and social determinants of health.
  • Act as an advocate for patients and families in complex interactions involving community agencies, payers, and healthcare providers.
  • Participate in community outreach events and recruit participants through strategic and effective outreach.
  • Enter data and maintain accurate documentation in work logs, medical records, calendars, data collection platforms, and files.
  • Maintain program records and submit reports in a timely manner.
  • Other health education, outreach, and community-building projects as assigned.

Qualifications:

  • Bachelor’s degree required.
  • Over 2 years of experience in a related field.
  • Understanding of effective primary prevention.
  • Experience working with diverse and underserved populations.
  • Bilingual Spanish/English or Khmer/English preferred.
  • Excellent speaking, writing, and analytical skills.
  • Ability to provide respectful, empathetic, and excellent customer service.
  • Ability to coordinate multiple projects and deadlines with attention to detail.
  • Ability to work and thrive in a fast-paced, dynamic environment; multi-task and shift priorities as needed.
  • Commitment to achieving significant prevention outcomes and reducing health disparities.
  • Ability to maintain a high level of professionalism and confidentiality.
  • Proficiency in Windows-based PC environment (word processing, spreadsheet, database, Internet).
  • Ability to work afternoon hours, evenings, and weekends.
  • A valid California Driver’s License, proof of insurance, and access to a car required.

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
