Position Overview:

The Case Manager is responsible for providing case management and success coaching services to young adults that are transitioning out of the justice system and participating in the grant program. This includes entering individuals into the CalJOBS system, entering activity codes, making case notes, providing wrap-around services and other services as needed. Your responsibilities will encompass case management, collaboration with staff, educators, families and other partners in the community to address the unique needs of underserved communities. Ensure participants have the necessary support, tools and materials to transition successfully into employment.


Key Responsibilities:


  • Case Management/Wrap-Around Services: Offer ongoing support services to a caseload of youth that are transitioning out of the justice system. Support will include direct services such as case management, career coaching, follow-up/job retention services, emergency cash assistance, transportation resources, bus passes, gas cards. Providing funding support to participants for training-related supplies, work tools, uniforms, and related program fees. Will refer participants to resources including services provided by partner organizations and local community-based organizations. 
  • CalJOBS Registration and Maintenance: Creating participant CalJOBS accounts, registration, application, eligibility and grant participation. Maintenance of caseload records includes entering activity codes for services that are provided to young adults, such as job search assistance, training, and counseling. Case note entry will include documenting the services that are provided to individuals and the progress that is made towards employment. 
  • Maintain Records and Data Entry: This includes setting up alerts for individuals on caseloads, such as WIOA Activity projected start dates, search lists of individuals who meet specific criteria, such as being eligible for a particular program or service. Entering Measurable Skill Gains (MSG) and Credential Attainment information for individuals who are enrolled in education or training programs.
  • Collaborate with stakeholders: Coordinate with families, juvenile counselors, parole officers, educators and other staff to support young adults and their career goals. Collaborate with industry professionals to stay updated on current trends and practices in relevant technical fields.
  • Provide other services as needed. This includes providing other services to individuals as needed, such as job search assistance and referrals to other services.
  • Instructional support. Provide instructional support by learning and becoming familiar with the Career Choices & Changes curriculum to effectively assist students throughout the program.


Qualifications

  • Associate/Bachelor's degree in social work, human services, or related experience
  • 1-2 years of experience working with young adults who are enrolled in workforce development programs, reentry or related experience.
  • Knowledge of CalJOBS is a plus.
  • Understanding of local community-based resources.
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Demonstrated commitment to diversity, equity, and inclusion.
