We are seeking a diligent and compassionate Intake Coordinator to become an essential part of our dynamic team. Job Responsibilities

  • Serve as the initial point of contact for clients preparing for an outpatient procedure
  • Conduct initial assessment interviews to gather relevant information and determine service needs
  • Accurately record and maintain clients' personal and confidential information in the organization's database
  • Coordinate with other team members to schedule services for clients and obtain necessary pre-procedure data
  • Provide clients with information about the services offered by the organization and guide them through the intake process
  • Follow up with clients to confirm appointments and provide reminders Liaise with healthcare providers, and other external agencies as necessary
  • Monitor and manage the intake process to ensure timely and efficient service delivery
  • Address and resolve any issues or concerns that arise during the intake process Develop and maintain a thorough understanding of the organization's programs and services to effectively match client needs
  • Collaborate with clinical or leadership staff to facilitate appropriate referrals and service planning
  • Participate in regular staff meetings and training sessions to stay current with organizational procedures and policies
  • Ensure compliance with all federal, state, and local regulations as well as organizational policies regarding client intake
  • Provide feedback and suggestions to improve the intake process and enhance client satisfaction.

Job Requirements

  • Minimum of 2 years of experience in a healthcare setting, in intake coordination or a similar role
  • Proficiency in electronic health records (EHR) and healthcare management software
  • Strong understanding of medical terminology Excellent communication and interpersonal skills
  • Ability to handle confidential information with discretion and professionalism
  • Strong organizational and time management skills
  • Ability to multitask and prioritize in a fast-paced environment
  • Detail-oriented with a focus on accuracy and efficiency
  • Experience in conducting initial patient assessments and coordinating with healthcare providers
  • Ability to work collaboratively with a multidisciplinary team to provide patient ­focused care
  • Knowledge of state and federal healthcare regulations
  • Basic CPR and First Aid certifications may be required
  • Flexibility to work various positions depending on the needs of the facility and a willingness to be trained and maintain the skills to cover those needs
  • Bilingual abilities are a plus
