Job Summary:
The Bilingual Medical Case Manager (MCM) is responsible for optimizing healthcare outcomes for HIV-positive individuals by providing comprehensive case management services. This role involves developing and implementing individualized care plans, coordinating care, and advocating for clients to ensure access to necessary medical and support services. Medical Case Management is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum.
Duties & Responsibilities:
- Provides case management services to all assigned clients.
- Completes client intakes, needs assessments, and care plans upon initial contact with new clients
- Maintains regular contact with all clients and completes needs assessments and care plans with clients, with a goal of client self-sufficiency.
- Responsible for ADAP/THMP program applications and recertifications and reporting
- Conduct comprehensive client assessments to identify healthcare needs and develop tailored care plans.
- Coordinate and monitor medical appointments, treatments, and medication adherence.
- Provide health education, counseling, and support to improve treatment outcomes.
- Link clients to essential community resources and services.
- Maintain accurate and up-to-date client records, including electronic health records.
- Collaborate with healthcare providers and other stakeholders to ensure optimal care delivery.
- Adhere to program guidelines, regulations, and quality assurance standards.
- Provide client advocacy and support as needed.
- Other duties as assigned
Qualifications & Skills:
- Bilingual proficiency in English and Spanish.
- Strong organizational and time management skills.
- Excellent written and verbal communication abilities.
- Experience in case management, preferably in a healthcare setting.
- Proficiency in electronic health records and data management systems.
- Knowledge of and sensitivity to HIV/AIDS issues, cultural/ethnic issues, diverse populations, LGBTQIA+ issues, substance use disorder issues, opioid use disorder, and grief and bereavement.
Education, Licensure & Experience:
- Bachelor’s degree in social work or related field is required.
- A minimum of 1 year working or lived experience with LGBTQIA+ community is required.
- A minimum of 1 year work or lived experience working with people living with HIV/AIDS or chronic health condition is required.
Work Schedule:
- Regular schedule Monday to Friday 8AM - 4:30PM
- Flexible schedule, if required
- Travel occasionally

Requirements:
- Must maintain a valid driver’s license and current automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive company owned vehicle or your personal vehicle during company business.

Must be able to lift and maneuver a minimum of 25 pounds using proper safe lifting techniques.
