Job Title: Case Manager

Job Summary:

We are seeking a skilled Case Manager to join our team at Laurel Ridge Treatment Center. As a Case Manager, you will play a critical role in ensuring that our patients receive high-quality, compassionate care. Your expertise in utilization review, patient advocacy, and clinical case management will enable you to make a meaningful impact on the lives of our patients and their families.

Responsibilities:

  1. Conduct thorough admission and continued stay reviews to ensure that patient care meets clinical needs and reduces financial risk to the facility.
  2. Participate in measures to improve the delivery and utilization of care, and evaluate the effectiveness of these initiatives.
  3. Enter all authorizations into the MS4/Midas system daily.
  4. Advocate for internal and external customers, including patients, families, physicians, treatment teams, and managed care organizations.
  5. Work in partnership with physicians through rounds and staffing to facilitate compassionate patient communication and advocacy, effective treatment team planning, and timely discharge planning.
  6. Provide a strategic link between managed care organizations, treatment team goals, and the financial aspects of care.

Qualifications:

To be successful in this role, you will need:

  • Good verbal and written communication skills.
  • Ability to maintain confidentiality.
  • Knowledge of Joint Commission, CMS, and Tricare standards.
  • Computer skills and knowledge of basic case management and utilization review principles.

Education and Experience:

A Bachelor's degree in a related field is preferred, or a High School Diploma with 5 or more years of experience in the mental health field and utilization review.

Licensure/Certification: LVN, RN, LPC, or LMSW is preferred.

Experience: At least two years' experience in a mental health setting with previous experience in case management/utilization review and discharge scheduling is preferred.

Physical Requirements:

This role requires:

  • Correctable vision and hearing.
  • Ability to occasionally lift pounds and carry medical records.
  • Ability to write and record information, and accurately read medical records.
  • Ability to spend % to % of time standing or walking between units, and % to % of time sitting at a desk.
  • Infrequent telephone or public face-to-face contact, but ability to calmly handle frustrated or angry clients.
