Job Title: Case Manager

Job Summary: We are seeking a skilled Case Manager to join our team at Laurel Ridge Treatment Center. As a Case Manager, you will be responsible for coordinating patient care to optimize outcomes, advocating for patients and facilities, and promoting quality care and documentation.

Responsibilities:

  • Conduct admission and continued stay reviews to ensure that care meets the clinical needs of clients and reduces financial risk to the facility.
  • Participate in measures to improve the delivery and utilization of care and to evaluate the effectiveness of these.
  • Enter all authorizations into the MS4/Midas daily.
  • Advocate for internal and external customers, including clients, families, physicians, treatment teams, and managed care organizations.
  • Work in partnership with physicians through rounds and staffing to facilitate compassionate patient communication and advocacy, effective treatment team planning, and timely discharge planning.
  • Provide a strategic link between managed care organizations, treatment team goals, and the financial aspects of care.

Qualifications:

Requirements:

  • Good verbal and written skills.
  • Ability to maintain confidentiality.
  • Some knowledge of Joint Commission, CMS, and Tricare standards.
  • Computer skills and knowledge of basic case management/utilization review principles.

Preferred Qualifications:

Education: Bachelor's degree in a related field preferred or High School Diploma with 5 or more years of experience in mental health field and utilization review.

Licensure/Certification: LVN, RN, LPC, or LMSW preferred.

Experience: At least two years' experience in mental health setting with previous experience in Case Management/Utilization Review and discharge scheduling (preferred).

Physical Requirements:

Correctable vision and hearing.

Must be capable of occasionally lifting pounds and carrying medical records.

Must be able to write and record information, and accurately read medical records.

Must be able to spend % to % of time standing or walking between units. % to % of the time is spent sitting at a desk.

Telephone or public face-to-face contact is infrequent, but must be able to calmly handle frustrated or angry clients.
