Position Overview: The AFC Outreach Assistant primarily focuses on raising awareness of the GLSS AFC program and targeting referrals of consumers who are a good match for the GLSS AFC program. Outreach to medical providers and educating providers on the importance of timely completion of the PCP Order Form. Provide medical information needed to determine clinical eligibility for the AFC program. Provide information about available services and community resources to consumers who may not be a good match for the GLSS AFC program. This position requires a thorough knowledge of available community services and community resources.

Position Responsibilities: Raising the awareness of existing GLSS services and targeting referrals of consumers who are a good match for the GLSS AFC program. Educating referral sources on what GLSS AFC does. Obtain and confirm consumer demographic information as appropriate. Assist consumers who are not a good fit for the GLSS AFC program, to identify and access services and support through other Agencies or programs. Make referrals to other agencies for additional services and support as needed.

Qualifications:

  • High School Diploma is required. Associate's or Bachelor's Degree is preferred.
  • Minimum of (2) two years of experience coordinating referrals and providing administrative support to Clinical and/or Case Management staff
  • Bilingual Spanish skills required
  • Knowledgeable of the elder care network
  • Possess the knowledge and skills necessary to work successfully as a member of an interdisciplinary team
  • Successfully complete the AFC Outreach training program
  • Solid written and verbal skills, and the ability to develop and maintain effective interpersonal relationships with staff members, consumers and partners
  • Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook
  • Ability to process information accurately
  • Ability to work both independently and as part of a team
  • Professional demeanor with the ability to handle confidential information and documents with discretion

Hours Per Diem - Monday through Friday
