We're looking for someone to join our team as a Case Manager - who:

Non-medical case management is the provision of a range of client-centered activities focused on improving health outcomes in support of the HIV care continuum. Activities may be prescribed by an interdisciplinary team that includes other specialty care providers. Non-medical case management includes all types of case management encounters (e.g, face-to-face, phone contact, and any other forms of communication).


Essential Functions:

  • Ensures that all HIV clients (PLWHA) have access to primary health care. Coordinate case management process from patient's entry into care and follow up care.
  • Provides intake and assessment of all HIV patients (PLWHA) as required by State, Federal, and program requirements. Ensure that appropriate forms are completed and accurate. Provides at minimum semi-annual caseload updates and annual reassessments as required by RW program.
  • Assesses needs of patient/family. Develops individualized client care plan with clients input to set and achieve short term and long-term goals toward improved HIV and related clinical outcomes and quality of life. Update care plan to implement changing patient care needs & case management goals. Monitors and updates, care plans, as needed/required.
  • Appropriately identifies patients who need case management, based on the complexity of their needs and Program eligibility.
  • Coordinate care and resources with medical providers, nursing and clinic support staff, other case managers, other team members and community resources to achieve optimal client healthcare outcomes.
  • Monitors patient progress, compliance and access to care. Provide follow up to those lost to care.
  • Routinely monitors and assesses quality of care, patient/family satisfaction with services, adherence, needs and effectiveness/appropriateness of case management services.
  • Conducts comprehensive psychosocial assessments for people with HIV/AIDs seeking services at intake and completes and documents client data for quality tracking and program accuracy.
  • Assess client's mental health needs and provides access and linkage to crisis intervention as necessary. Non-medical case managers are responsible for completing lethality assessment referrals to Program Substance Abuse Case manager/counselor.

You'll be successful with the following qualifications:

  • High school diploma or GED required.
  • Completion of a Medical Assistant program or Certified Medical Assistant highly preferred.
  • Bachelor's level training in Business Administration, Public Administration, or Public Health preferred. Equivalent combination of experience and education will be considered.
  • Experience in program management in health care, social service, or community service setting.
  • Two (2) years' experience in management preferred. Health care organization experience highly preferred.
  • Ability to work well and communicate with varied community groups and individuals.
  • Bilingual (English/Spanish) preferred.
  • Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval.
