We're looking for someone to join our team as a HOPWA Case Manager who:

The HOPWA Case Manager provides housing case management aimed at providing Housing specialist services. HOPWA case manager will provide housing assistance and supportive services for low-income HOPWA eligible clients to reduce the risk of homelessness. Case manager will collaborate with the client, and various health care providers, housing collaboratives, to facilitate client access to those health and social services which enhance the client’s health and housing stability. Case management services will include client advocacy, monitoring client care to remove housing barriers, addressing client and family needs, providing services to client through coordination with clinical and community resources settings.

Essential Functions:

  1. Coordinate case management process from patient's entry into care. Provide continuously follow up with patient care.
  2. Complete intake and assessment of HOPWA clients, as required by State, Federal and program requirements. Ensure that appropriate forms are completed and accurate. Provides at a minimum semi-annual client caseload updates and annual reassessments as required by HOPWA program.
  3. Assesses needs of patient/family. Develops individualized housing care plan with clients input to set and achieve short term and long-term goals toward improved and sustaining housing. Monitors and updates, care plans, as needed /required.
  4. Coordinate care and resources with medical providers, nursing and clinic support staff, the patients care team, and community resources to achieve optimal client healthcare outcomes.
  5. Monitors patient progress, compliance and access to care. Provide follow up to those lost to care.
  6. Routinely monitors and assesses quality of care, patient/family satisfaction with services, adherence, needs and effectiveness/appropriateness of case management services.
  7. Function as coordinator to central and primary access points for financial assistance programs including but not limited to Ryan White Treatment and client Housing services.
  8. Exhibits familiarity with resources for PLWHA, community support services, social service agencies and other available resources. Collaborates, as needed, for the benefit of the client.
  9. Maintains compliance with HOPWA funding requirements.
  10. Participates in program Quality Management program and assists in achieving goals.
  11. Document ALL case management services provided for each HIV client in Electronic Medical Record.
  12. Other duties as assigned

You'll be successful with the following qualifications:

  1. Must have High school diploma or GED required.
  2. Must have experience with outreach, low-income families, diverse communities, community/migrant health.
  3. Bachelor’s level training in Business Administration, Public Administration, or Public Health preferred. Equivalent combination of experience and education will be considered.
  4. Experience in program management in health care, social service, or community service setting.
  5. Two (2) years’ experience in management preferred. Health care organization experience highly preferred.
  6. Ability to work well and communicate with varied community groups and individuals.
  7. Bilingual (English/Spanish) preferred.
