The intake coordinator is responsible for providing customer service assistance to prevent and resolve problems linked with treatment services throughout TSS and the greater Atlanta community.

 

Key Performance Indicators:

Referrals will be screened to determine eligibility for services

POE and client assignments are completed within 24 hours

Insurance will be verified by the 6th of each month

 

Office Maintenance

  1. Open and close the Atlanta office daily
  2. Retrieve referrals from BHL, fax, email, and phone
  3. Answer incoming calls and emails to Trinity Social Services and provide regarding treatment services and employment
  4. Scan/Upload documents into CareLogic Document Library
  5. Ensure demographic and financial information within CareLogic is accurate:
  6. Process Releases of Information

Assessments

  1. Obtain demographic and treatment history information and indicate on the appropriate documents within CareLogic
  2. Receive phone calls, faxes, and emails regarding requests for treatment services from referral sources
  3. Complete required documentation for Medicaid Clients
  4. Assign initial behavioral health assessments to Assessors based on availability and service area
  5. Enter referral statistics in Management Worksheet
  6. Update the Referral Tracker tab in Management Worksheet
  7. Assign assessments to the Assessor
  8. Upload authorization information into CareLogic for initial services
  9. Update staff relationships for assigned clients
  10. Receive consents via email and upload into Document Library

Hospitals/Hospital Discharge

  1. Receive phone calls from families, clinicians & discharge planner
  2. Document behavioral health hospital admissions on Memo to Chart with Hospital activity code
  3. Document hospital discharges on Memo to Chart with Hospital discharge activity code

Clinic

  1. Schedule appointments for Psychologists, Nurse Practitioner and Nurse
  2. Create orders for services associated with treatment at least two days before clinic
  3. Review consent documents with clients in need of updates and/or missing consents, obtain signatures, scan/upload documents into the Document Library
  4. Obtain Release of Information for communicating with PCP
  5. Review and obtain signatures on Treatment Plans
  6. Disseminate client surveys and retrieve completed surveys

 

Discharges

  1. Notify clinicians with inactive insurance of the need to close/transition chart
  2. Conduct follow-up satisfaction surveys with individuals who have been discharged
  3. Complete administrative discharge on individuals with open charts but no services
  4. Complete discharge summary associated with clients

 

Qualifications:

Bachelor’s Degree from an accredited college or university. A minimum of two to five years of counseling experience in the social services industry with proven results is required; behavioral health is highly preferred. Experienced in customer service, scheduling, and referral processing in a social services environment. Possess an equivalent combination of education and experience and the ability to communicate clearly and concisely in writing and orally. Has general knowledge of community resources and the ability to aid in the company’s success and growth.
