Job DescriptionJob Description

Psychiatric Hospital located in Queens, NY is looking to add a dynamic and reliable Social Worker (LMSW) to its growing case management department. The social worker's role is to determine the need for financial and medical assistance, provide supportive counseling, arrange concrete services, participate in counseling, and determine the suitability for all treatment modalities. This is a Long-term contract role.

Qualifications:

  • Bachelor's or master’s degree from a program accredited by the Council on Social Work Education. Complete a course study with a specialization in clinical practice.
  • Prior experience with patients in a hospital or Psychiatric facility
  • New York State Licensed Social Worker License

Responsibilities:

  • Evaluate patients within assigned areas or social work discipline. Perform ongoing assessment of all patients' social, psychological, environmental, and financial factors.
  • Develop a social work treatment plan and coordinate the multidisciplinary team’s development in program evaluation, development, and implementation of treatment and long-term plans.
  • Counsel patients, family members, significant others, and staff in adjustments to chronic illness, changes in lifestyle, and choice of treatment modality.
  • Represent patients to community agencies through referral and follow-up. Ensure patient access to appropriate resources and develop relationships and referrals.
