Your Role

  • Provide outreach services to individuals facing mental health challenges, delivering in-home interventions as needed.
  • Conduct psychosocial evaluations in collaboration with team members to determine the mental health needs of clients and their families.
  • Develop and implement short-term service plans in partnership with an interdisciplinary team.
  • Respond to client needs promptly, ensuring timely travel to client locations.
  • Act as a liaison with community agencies to facilitate seamless program operations.
  • Maintain comprehensive care continuity, from counseling to long-term treatment referrals.
  • Document case histories and maintain accurate records in accordance with program requirements.
  • Encourage clients to engage with mental health services through supportive interventions.
  • May provide clinical supervision to junior staff or graduate students in social work.
  • Must hold a current registration as a Licensed Master Social Worker in New York State.
  • Possession of a valid driver's license may be necessary.
  • A minimum of one year of experience in a healthcare setting is required.
  • Ability to navigate stairs to access client residences is essential.
  • A Master's degree in social work from an accredited institution is required.
