Job Title: Licensed Master Social Worker

We are seeking a highly skilled and compassionate Licensed Master Social Worker to join our team at the Charles B. Wang Community Health Center. As a key member of our mental health team, you will be responsible for providing high-quality psychosocial assessments, developing treatment plans, and delivering evidence-based interventions to our patients.

Responsibilities:
  • Conduct comprehensive psychosocial assessments, including diagnostic interviews, to identify patients' needs and develop individualized treatment plans.
  • Provide psychotherapeutic interventions, including counseling and psychotherapy, to patients with a range of mental health conditions.
  • Collaborate with primary treatment teams to ensure comprehensive care and coordination of services.
  • Provide crisis intervention and case management services to patients in need.
  • Maintain accurate and up-to-date treatment records in accordance with Health Center guidelines.
  • Participate in agency Quality Assurance activities and case conferences to ensure high-quality care.
  • Attend departmental meetings and contribute to the development of policies and procedures.
Requirements:
  • Master's Degree in Social Work from an accredited social work school.
  • New York State Social Worker License required.
  • Basic CPR (BCLS) Certification required (Health Center will provide if needed).
  • Effective verbal and written communication skills.
  • Bilingual in English/Chinese (Mandarin and/or Cantonese) or English/Korean.
