The City of New York is seeking a highly skilled Social Worker to join the Bureau of Tuberculosis Control. As a key member of our team, you will play a critical role in ensuring that patients and their contacts receive necessary social and mental health services.
Key Responsibilities:
- Supervise and Train Social Workers: Provide guidance and oversight to junior social workers, assisting with ongoing training and staff development to ensure they are equipped to provide high-quality services.
- Case Management and Referrals: Respond to referrals from clinic providers, conduct thorough assessments to identify social service needs, and refer patients to community agencies to meet their needs.
- Interviews and Counseling: Conduct interviews with tuberculosis patients to identify needs for social services, including housing, food, and health insurance, and provide supportive counseling to ensure adherence to TB treatment and care.
- Documentation and Reporting: Accurately document interviews and outcomes of social service referrals in our TB database systems, and analyze and prepare reports related to referrals, linkages to social support in the community, and social service needs of patients.
- Quality Assurance and Collaboration: Participate in case reviews and quality assurance measures to ensure high-quality services, and collaborate with other teams to ensure seamless delivery of services.
Requirements:
- Education: A Master's Degree in Social Work from an accredited school of social work is required.
- Licensure: A valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license issued by the New York State Department of Education is required within one year of appointment.
- Residency: New York City residency is generally required within 90 days of appointment, with certain exceptions for City Employees with 2 continuous years of service.
