  • Collaborate with primary treatment teams to ensure comprehensive care and coordination of services.
  • Provide crisis intervention and stabilization services to patients in crisis.
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement activities, including case conferences and peer review, to ensure high-quality care and services.
  • Stay up-to-date with best practices and evidence-based treatments in social work and mental health.

Requirements:

  • Master's Degree in Social Work from an accredited social work school.
  • New York State Social Worker License required.
  • Basic CPR (BCLS) Certification required (Charles B. Wang Community Health Center will provide if needed).
  • Effective verbal and written communication skills.
  • Bilingual in English/Chinese (Mandarin and/or Cantonese) or English/Korean.
