Job Description:

The Patient Care Coordinator works within the Referral Department and is primarily responsible for securing Referral paperwork from Nursing Facilities and creating and maintaining the schedule for Psychology and Psychiatry providers. The Patient Care/ Referral Coordinator facilitates referral management working with providers, patients, and the health plans to share essential patient information and assist primarily in scheduling and care authorization.


Essential Duties and Responsibilities:

  • Verifying patients insurance coverage with Medicare, Medicaid, and Commercial Healthcare carriers.
  • Obtaining pre-authorizations from Health Insurance plans.
  • Answer incoming and make outgoing calls to secure referrals and authorizations from providers and insurance companies.
  • Ensure that referrals are addressed accurately and in a timely fashion.
  • Ensure all authorizations are properly documented in the system.
  • Scanning documents into the EMR system. 

Qualifications:

  • Medical insurance experience.
  • Experience verifying patients insurance coverage with Medicare, Medicaid and Major Health plans
  • Experience obtaining pre-authorizations from health plans.
  • Able to multi-task, personable, and have excellent customer service skills.
  • Self-starter with a positive teamwork attitude, well organized and detail oriented.
