We are hiring for 2 case managers to work at an ADHC in Santa Ana, hours are 8AM - 5PM.  Candidates must have BA degree and geriatric case management experience.  This is temp with potential of temp to hire.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Conducts client-specific assessment of needs; identifies problems and establishes client-centered immediate requirements and long-range goals.
2. Arranges and coordinates a network of supportive services and entitlements (formal and informal) consistent with mutually-developed care plan.
3. Maintains required records and reports in compliance with department, agency, local, state and federal requirements.
4. Schedules and attends meetings, develops agendas, takes minutes and conducts follow up on required items.
5. Assists in administrative projects for the department(s).
6. Represents the program with staff and clients and in networking meetings, speakers’ bureaus, and trainings.
7. Follows up on client-related issues.
8. Supports special projects as requested, including providing support for the Care Transitions program.
9. Assumes responsibility for all case records and monthly statistics.
