Job Summary

We are seeking a skilled Case Manager to join our team at LHH Recruitment Solutions. As a Case Manager, you will provide comprehensive case management services to underserved individuals, connecting them with appropriate resources and services to address barriers and promote housing stability and self-sufficiency.

Key Responsibilities
  • Conduct intakes and assessments to determine participants' needs and develop individualized service plans (ISPs), schedule interdisciplinary meetings.
  • Connect participants with resources and services to address barriers, collaborate with community partners, shelters, and other agencies to facilitate access to resources and coordinate appropriate care.
  • Maintain accurate and up-to-date documentation of participant interactions, progress, resources, and referrals.
  • Advocate on behalf of participants to ensure access to essential services and support their goals for housing stability and self-sufficiency.
  • Participate in team meetings, case conferences, and ongoing training sessions as required.
Requirements
  • Minimum 1 year experience in Case Management, Social Work.
  • Comfortable working with homeless population.
  • Strong interpersonal skills and the ability to build rapport with diverse populations.
  • Excellent communication and problem-solving abilities.
  • Compassionate and non-judgmental approach to working with clients.
