Job SummaryWe are seeking a Part-Time Case Management Support Specialist to join our team at our Community Resource Center. The successful candidate will provide case management support to participants, assist Case Managers with case coordination, and perform various administrative tasks.
Key Responsibilities- Provide case management support to participants, including checking in participants and assisting with case coordination and documentation.
- Assist Case Managers with communication with appropriate agencies and preparation and distribution of reports.
- Conduct risk/need assessments and create individual treatment plans for each assigned participant.
- Facilitate evidence-based CBT programs and other life skills programs to participants in groups each week.
- Develop and monitor the participant's progress relative to their Case Management Plan.
- High school diploma required; Baccalaureate Degree in Social Services preferred.
- Bi-lingual (English/Spanish) skills preferred.
- College coursework in Human Services or business discipline preferred.
- Minimum of six months experience in Human Services field with direct participant services preferred.
- Good customer service skills.
- Effective communication skills with internal and external contacts at all levels.
- Good organizational skills and attention to detail.
- Solid computer skills and proficiency with MS Word and Excel.
