Service / Care Coordinator PAY RATE: $18-$20 per hour

Service / Care Coordinator Job Description:
• Inbound call center environment
• Assist and educate Medicaid members on benefits and Services such as PCP Changes, ID Cards and Coordinate transportation and other services.
• Update member record as needed
Service / Care Coordinator Required Skills:
· Call Center, Medical Background, Working with the Aging, Blind and Disabled Population]
· Medicare / Medicaid experience required
· Call center experience is a must
· Strong documentation skills
· Excellent computer skills
· High School Diploma or GED
Service / Care Coordinator Preferred Skills:
· Service Coordination, Case Management or Nursing experience preferred.
· Background in Managed Care
· Familiarity and experience working with Medicare or Medicaid plans
· Degree preferred
