As a Care Manager, you will have a consistent workload and will be responsible for an assigned caseload of OPTIONS and Caregiver Support Program consumers, assisting them with receiving needed services and providing on-going monitoring. This position also requires some on-call, which is extra compensation. The successful candidate must be precise, thorough, accountable, and driven to deliver high-quality work.
Applicants will possess a Bachelor’s degree with a social welfare major with coursework in sociology, social welfare, psychology, gerontology, or other related social sciences with one year of experience in public or private social work. Experience in direct aging is a plus. Excellent verbal and written communication skills and advanced computer skills are required. Must have a valid PA driver’s license and current auto insurance.
The hourly rate is $17.50/hour which is based on a 37.5 hour work week, Monday through Friday. We offer an excellent benefit package that includes a generous amount of paid time off, a retirement plan with an annual contribution rate of 6%, and group health insurance with a low monthly premium plus dental, vision, and supplemental insurances.
