Description of role / Job Summary:

As part of the SLS (Supported Living Services) Senior Management Team, the Supervising Case Manager (SCM) is responsible for overseeing and supporting assigned SLS Case Managers, SLS Supervisors and Caregivers / Direct Service staff. The SCM is responsible for Staff supervision, leadership, staff development, training, performance improvement, implementation and management of key services in accordance with policies and procedures. The SCM effectively interacts with the individuals served, and their circle of support on the processes of assessment, planning, intervention, implementation and evaluation. Under the direction of the Program Director, the Supervising Case Manager (SCM) establishes and maintains good client relations and service by directing, coordinating and managing SLS Case Managers, SLS Supervisors & SLS Direct Service Staff. The SCM ensures coverage of shifts and serves as a backup for Staff temporarily absent. It is expected that the Supervising Case Manager (SCM) will spend more than 80% of his or her time supervising and developing staff.

Essential Functions:

  • Schedule SLS Case Managers, SLS Supervisors & SLS Direct Services Staff
  • Act as first responder in the field to emergencies and report special incidents according to applicable laws and regulations
  • 24 hour on call response, as applicable
  • Participate and provide training in the development of the program plan as well as client pre-annual assessment as needed
  • Communicate on a regular basis with regional center, residential facilities, families and other services providers and agencies, as needed and or directed
  • Responsible for supporting the selection, training and coaching of SLS Case Managers, SLS Supervisors and SLS Direct Services Staff
  • Responsible for reviewing, evaluating and supervising assigned SLS Case Managers, SLS Supervisors and SLS Direct Services Staff
  • Plan, manage, coordinate, track, and evaluate assigned Staff efforts to achieve the programs objectives and individuals goals
  • Coordinate with the development and implementation of the individuals IPP objectives and or ISP objectives for which the program is responsible
  • Responsible for supporting the development & maintenance of SLS Training Materials & Resources
  • Maintain monthly calendar of activities for individuals/group, as pertain to assigned program
  • Coordinates and shares the interdisciplinary treatment planning, individual assessment, intakes, meetings, and participates in client quarterly/semi-annual/annual meetings
  • Performs other related duties and assignments as required

Tasks (partial list):

  • Supervise, train, and provide guidance to assigned SLS Case Managers, SLS Supervisors, SLS Caregivers / Direct Services Staff in accordance with State, Federal, Regional Center and funding authority regulations, NorCal Individual Care, Inc personnel policies and Program Procedures & Operations Manuals
  • Supervise and participate as part of an interdisciplinary team in the development of each client’s Individual Support Plan (ISP); assess ongoing effectiveness of achieving objectives
  • Supervise up to 4 SLS Case Managers and or SLS Supervisors
  • Manage up to 24 Client Cases
  • Supervise and assist with direct service for each client, when needed
  • Supervise and serve as off-hours (weekends) contact on rotating basis with management team
  • Supervise and actively help clients find permanent housing
  • Supervise and help clients manage the clients’ IHSS workers
  • Supervise and complete daily, weekly, monthly, and annual documentation accurately, and as required by NorCal Individual Care, Inc, State and Federal regulations; ensure timely submission
  • Supervise and maintain client information, files & documents according to established standards and protocol at NorCal Individual Care, Inc
  • Supervise and maintain requisite client documentation (i.e., calendars, ISPs, etc.) in clients’ homes, on company systems & company approved devices as appropriate
  • Supervise and monitor condition of clients’ homes, and facilitate all repairs and required maintenance of a client’s home
  • Supervise and take responsibility for the following of clients’ budgets and ensuring payment of bills
  • Supervise and support maintenance of clients’ Medicaid eligibility by assisting staff with scheduling and attending all Medicaid appointments and providing proper documentation, when needed
  • Supervise and maintain professional communication with families, Regional Center Service Coordinators, Case Managers, Behavioral Consultants, health care personnel, schools, landlords, employers, etc.
  • Supervise and participate and/or facilitate in all meetings as needed
  • Supervise and manage Clients schedules
  • Supervise SLS Case Managers & SLS Supervisors Time-off & PTO
  • Supervise SLS Case Managers & SLS Supervisors Time & Attendance (Time Card) Hours
  • Supervise and manage Direct Services Staff Time-off & PTO
  • Supervise and manage Direct Services Staff Time & Attendance (Time Card) Hours
  • Assist the Program Director in Managing & Supervising Staffing, Scheduling & Training
  • Responsible for completing I-9 Forms for new hires
  • Assist the Program Director in various projects
  • Respond promptly and appropriately to on-call situations

Skills and requirements:

  • 5 years of experience in a human services delivery system, preferably in comparable programs serving developmentally disabled adults
  • Completion of at least 4 years of post-high school education in a human services-related field or specialization
  • Minimum of 4 years of experience managing line Staff
  • Minimum of 2 years of experience supervising Managers
  • Minimum of 2 years of professional experience in a SLS program preferred
  • At least 18 months’ experience and/or training on working with clients with behavioral issues
  • Proven leadership and team building skills
  • Proficient in Computer applications
  • Successful background check
  • Valid CPR Certification
  • Valid First Aid Certification
  • Valid California Driver’s License
  • Proof of Auto Insurance
  • Reliable transportation

Education and Experience:

  • A Bachelor's degree in human services, or equivalent preferred
  • DSP 1 and 2 certification preferred
  • CPR Certificate
  • First Aid Certificate
  • 5 years minimum experience in a human services delivery system, preferably in comparable programs serving developmentally disabled adults
  • Experience requirements can be waived if candidate possesses a Bachelor's degree in human services or equivalent

Work schedule:

As an Exempt salary position, the positions work schedule varies; can include weekend or overnight shifts when necessary. Applicant must be willing to be available to their assigned Managers and Staff before and after regular business hours. The applicant must also be available to provide coverage when there is a staff shortage.
