Public Health Nurse

Job Overview
We are seeking a dedicated and compassionate Community Health Nurse to join our team. The successful candidate will play a crucial role in promoting and protecting the health of individuals, families, and communities through evidence-based practices. You will work collaboratively with other healthcare professionals to provide preventative care, education, and treatment services to populations at risk.
Key Responsibilities
Health Assessment and Planning:
- Conduct comprehensive health assessments for individuals and communities.
- Identify health needs and risks, and develop appropriate care plans.
- Implement strategies to address social determinants of health.
Direct Patient Care:
- Provide clinical services including immunizations, wound care, screenings, and health counseling.
- Administer medications and treatments as prescribed.
- Manage chronic diseases and coordinate care for patients with complex health needs.
Health Education and Promotion:
- Develop and deliver health education programs and workshops.
- Educate individuals and groups on health maintenance and disease prevention.
- Create and distribute educational materials to the community.
Community Outreach:
- Build and maintain relationships with community partners and stakeholders.
- Participate in community health fairs and other outreach activities.
- Advocate for community health needs and resource availability.
Data Collection and Reporting:
- Collect, analyze, and report health data to monitor community health trends.
- Maintain accurate and confidential patient records.
- Contribute to public health reports and grant applications.
Emergency Preparedness and Response:
- Participate in the planning and execution of public health emergency responses.
- Provide crisis intervention and support during public health emergencies.
Qualifications
Education:
- Bachelor of Science in Nursing (BSN) required; Master’s degree in Nursing or Public Health preferred.
Experience:
- Minimum of 2 years of nursing experience, preferably in community or public health.
- Experience in case management, patient education, and chronic disease management.
