Responsibilities:
- Schedule and coordinate clinical appointments for patients requiring home health care services.
- Communicate effectively with patients, their families, and healthcare providers to ensure all appointments are scheduled promptly and accurately.
- Maintain accurate records of patient information, appointments, and communication logs.
- Assist patients with inquiries, provide information about services, and address any concerns or issues in a professional and compassionate manner.
- Collaborate with other members of the healthcare team to ensure seamless coordination of care for patients.
- Follow up with patients to confirm appointments and provide reminders as needed.
- Adhere to all privacy and confidentiality regulations when handling patient information.
- Provide administrative support as needed, including data entry, filing, and documentation.
Qualifications:
- High school diploma or equivalent required; associate’s or bachelor’s degree preferred.
- Previous experience in a healthcare setting, preferably in a patient services or customer service role.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
- Strong organizational skills and attention to detail.
- Ability to work independently and prioritize tasks in a fast-paced environment.
- Proficiency in Microsoft Office and other relevant software applications.
- Knowledge of medical terminology and healthcare regulations preferred.
- Compassionate and empathetic attitude towards patients and their families.
