Position Summary

Social Workers provide psychosocial assessments, early interventions and support for people going through all kinds of difficulties in life by helping patients in developing skills to cope with the impact of illness and continuing care issues.

Essential Job Functions

  1. Complete and maintain documentation in a timely and accurate manner according to federal, state and center guidelines

  2. Serves as an expert, acts as a liaison between patients and community resources.

  3. Develop service plans and conduct reviews as needed throughout the duration of treatment.

  4. Monitor referrals made to community-based organizations, medical care and other services to support the members’ overall care management plan.

  5. Collaborate with a variety of health professionals in the patient’s circle of care including but not limited to primary care providers, care nurses, care coordinators, community health program workers, and other specialists in weekly case review meetings.

  6. Provide group interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

  7. Connect patients and family members with community and social resources.

  8. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence.

Supervisory Responsibilities

This position has no supervisory responsibilities.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.

Required Education and Experience

  1. Relevant: 1 year (Preferred).

  2. Bachelor’s degree in social work from accredited program.

  3. BLS certification.

  4. Flexible scheduled weekdays-weekend.

  5. Good communication and listening skills.

  6. The ability to show empathy and compassion.

  7. Strong leadership qualities.

  8. Able to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families.

  9. Ability to travel in assigned region.

Preferred Education and Experience

  1. Relevant or any other job-related vocational coursework.

  2. Bilingual Spanish and English.
