Summary:

The Case Management Program Manager is a member of the leadership team at Great Bay Services and oversees the case management program in Sanford, Maine. The Case Management Program Manager will be responsible for the management and oversight of the case management team with the support of a contracted clinical supervisor.

Case Management (CM) offers many services to individuals with disabilities. The program seeks to connect individuals with their communities so they may live as independently as possible. Individuals 18 and older will regularly schedule meetings with their case manager and collaboratively create an individualized yearly plan. Case Management provides individualized advocacy, planning, and coordination of services for clients.

Responsibilities:

  • Supervise Case Managers in the Sanford office
  • Monitor all aspects of CM work performance
  • Provide routine supervision to CMs individually and group via monthly staff meetings
  • Hire and coordinate training for new CMs as needed
  • Monitor and timely respond to referrals for CM services through Great Bay Services
  • Attend supervisory meetings and trainings as scheduled by DHHS OADS
  • Distribute and communicate information to the CMs to ensure they have current information and guidelines
  • Work closely with CMs and provide feedback to Program Manager to improve the quality-of-service delivery
  • Advocate for and coordinate provision of services including medical, housing, employment, day services, and community inclusion
  • Maintain accurate case records of assessment, activities, and plans; writes summaries, reports, letters, and memos
  • Serve as Representative Payee for individuals
  • Become Subject Matter Expert in Evergreen and other pertinent programming

Qualifications and Requirements:

  • Bachelor’s Degree or higher in social services, public health, or other related field
  • Registered Nurse and Licensed Independent Clinical Social Workers Preferred
  • Minimum of four (4) years’ experience providing services to persons with intellectual disabilities and/or autism
  • Experience leading a team
  • Knowledge of the public education system in Maine (including flexible funding and family-focused service provision)
  • First Aid & CPR Certified (certification training will be provided to candidates without it)
  • Valid Driver’s License
  • Carry Mandatory Auto Insurance of no less than 100K/300K
  • Must be highly ethical, motivated, dependable, and organized
  • Contribute to a culture of Safety, Passion, Accountability, Community, and Empathy
  • Must pass routine criminal background check, DHHS background check (including Child Protective Services and Adult Protective Services), and motor vehicle check background check
  • Evidence of commitment to the mission of Great Bay Services
